I would describe only what I observed. For example: "I am calling about Ms. Carter. Today she seemed weaker than usual when standing from the chair, and she ate less than normal at lunch. I helped her sit safely, offered water according to the care plan, and stayed nearby while she rested. I am not sure what is causing the change, so I wanted to report it and ask what you would like me to do next."
