I would explain that reporting unsafe or out-of-plan requests protects everyone. I would help them write a factual note, identify what part of the request is outside the care plan, and contact the supervisor promptly. I would also coach the wording: "I want to be helpful, but I need approval before doing tasks outside the care plan." This builds confidence and keeps the process safe.
