I would coach with examples, not criticism alone. I would explain that notes should include care completed, client response, changes observed, refusals, and actions taken. I might compare "client was fine" with "client ate half of lunch, walked with assistance, and refused shower; supervisor notified." Then I would review the next few notes and give feedback until the standard is clear.
